Property / Casualty Incident Reporting

Please follow this process for reporting property / casualty incidents (i.e., vehicle accidents, patron injury, property damage):

  1. Fill out the Incident Form as thoroughly as possible. The more information we have, the easier for us to assist you.
  2. Upload all correlating documents you have regarding the incident:
    • Police report
    • Photos of any damage
    • Quotes / Invoices for repairs or medical bills
